No last name

In FORM 47sk one thing is not clear.
I do not have last name in my PASSPORT how should I fill it in
VISA Form.

My name in Passport is - VIKAS . No last name.
In some places I have been using VIKAS PANCHAL as my name, e.g. in with
current employer.

In VISA Form in question no. 5, 7 and 9 are as follows:

Question 5 - Applicant's Full Name (as shown in your passport or travel
document):
Family Name _____________
Given Names _____________

             Question 7 -Other ways you spell your name:
                      Family Name _____________
                      Given Names _____________

Question 9 - Other names you are, or have been, known by
         _____________

In this case how these fields should be fill in?
Any help highly appreciated!

Also, One generic question - what is family name and what is
given names?

Simply, just mention your name and on question 9 say you are also
known as Vikas Panchal.
if DIMIA eventually asks you about your family name, you can then
explain. anyway you need to include a certified copy of your
passport's bio page so it will show "Vikas" as your full name. if it
makes you feel better, attach a letter to your application and explain
your situation.
DIMIA's priority is for you to be clear and honest about yourself.
