Does any of you who have received the request for medical examination
remember the medical examination form sent by Canadian immigration? We
have got only one page form with our photo for each of us. I feel that
the form should me more than a page because of the way cover letter
refers to the different sections of the form. Or does the doctor will
have rest of the form?

On the subject of medical exams.. I take it they mail you out the forms
when they want you to take the exam ? I'm not far off applying for PR
myself and have been wondering about the medical.. I thot it had to be taken
before you submitted your application..

Spousal applicants take the meds upfront, and the DMP has the forms, for
other categories the CHC mails out the forms.
