With respect to your pr application your personal circumstances have changed
and you must update your file and include your wife.
You have to pay the processing and right of permanent resident fees, you
have to obtain the medical forms for her to do her medical and you have to
supply the visa office with are the required information and forms including
photos.
It might delay the processing of your application however you have no
option, if you do not immediately alert the visa office your application
will be in jeopardy.
If you are from a country that requires a visa to come to Canada you may
experience difficulty in obtaining same, visa officers are very reluctant to
issue visitor visas to individuals who have an immigrant application in
process. There are legal precedents to allow visas but it is still at the
discretion of the visa officer.

I've collected all her certificates, got her photographs and arranged
for her fees.
Some more questions:
1. Is she required to go thru medicals and language test right away?
2. Should I also fill up some form? Or just a covering letter would be
sufficient?

You are *required* to report your marriage to the visa office
ASAP. Not reporting major changes in your situation (and getting
marriage is a very major change) can constitute
misrepresentation and can cause ineligibility of PR even losing
your PR status after getting it.
Your spouse will have to go through background check (police
clearance etc.) and medical checkup regardless whether your
spouse is included in your application or not.

You *have* to update your case, there nothing to think.

If you want your spouse come with you to Canada when you get PR,
then you will have to add your spouse to your application, then
yes, additional fee will be required. You will also need more
settlement fund.

requirement will be the same as if she applied with you from the
beginning. Please read through the form and instructions.

You have to add your spouse information, there is no if.

Just having PR application may add a little bit more
difficulties to get visa to Canada.
However, if you decide to not include your spouse to your
application, and then you get your PR visa then get your PR by
landing. Then it will be very-very difficult for your spouse to
get visitor visa to Canada.
