As I mentioned in my earlier post, you only need one certificate for
one country. The exception would be the US but you can still provide
one certificate and if CIC wants one issued by every state an applicant
had stayed in, they would request it.
Only send the certificate you have been provided by the state police in
which ever country you resided in and you do not need one for city by
city in the same country. Unless of course, you have lived in different
countries for more than 6 months duration.

Immigration Manuals or Guides are not the 'substantive law'. Based on
the fact that CIC might attach varying weights to 'PCCs' issued by
various countries, I think I would have to secure a 'separate' police-clearance-
cerfiticate from all the different 'districts' where I have resided in
Pakistan for 6-months or more. Based on my knowledge of Pakistan, police-clearance-
cerfiticates are issued by the 'senior-most' police officer of the
'district': 'provincial' or 'national' registries or databases may or
may not be maintained. As such, the Visa Office shall be in a perfect
legal position to expect a separate police-clearance-certificate for
each 'relevant' district.
Please note that I think that 'expectations' would vary from country to
country---depending on the manner in which the 'registries and/or
databases' are maintained. As the Applicant, the 'onus' lies on us to
present 'conclusive' evidence of 'non-criminality'. I think this is all.
