what to fill in To date for #11, #15 in SCHEDULE 1 BACKGROUND/ DECLARATION form

Can anyone help me for filling the form SCHEDULE 1 BACKGROUND/
DECLARATION.
I have a doubt about # 11 Personal History. what date should I fill in
"To" section for my current job. I am working with the same compay from
March 1. 2003 to till date. In From section I am filling 03-2003 but
what should I fill in To area.
should I leave it blank or fill todays day?
Same question for #15 Address: what should I fill in TO date area when
I am residing on same place.
Please help me to complete this.

Put in "present" in both areas.

you mean word "present" or current date like "01-2007"

Mr. Jim,
I have one more question regarding the experience letter. I worked in 3
companies including current company. I can attach the orginal
experience letter for my current company but for my old companies I
can't becouse I have only one copy. Question is will photo copy or
noterized copy of my old companies experice letter work?
