Help for filing Q13 in  IMM 5257 TRV application form

Hi,
I have a question regarding filing Q13 which says like this :
During the past five years have you or any family member accompanying
you lived in any other country than your country of citizenship or
permanent residence for more than six months.

The question is I am filling an application for me and my wife. For
last 3 years we are staying in USA, but we have gone to India twice in
the last 3 years. So do I have to fill out all the entry and exit to
USA in last 3 year or just the last entry to USA. If I have to fill
the last entry to USA in "From" what will be the date in "To". Or If I
have to fill all entry and exit,  I don't see enough space over
there,  only 3 lines are there to fill this information.

Any help is appreciated.

The question refers to stays of 6 months or longer so if you were just
visiting India for less than that do not report those stays.

I think Jim's answer is correct if you have USA Citizenship or a green
card.

If however you are an Indian Citizen, and a temporary visa holder in
the USA, then the time in the USA must be reported on that form. Just
print a second copy of the page and add it to the set of forms to fit
it all in.  The most recent "to" is "to present" or the date on which
you signed the  forms.

Hi,
I have one more question, I am mailing my application to Canada
Embassy at Los Angeles. Do I end to send some kind of cover letter
with my application?
If yes, where can I get a sample cover letter.

You don't really need a cover letter unless there is something that
you want to tell them that you did not get a chance to on the form.

If there is something that you want to add about the reason for the
visit, then go ahead and add a letter, but don't add one if it adds
nothing to the officer's decision making process.

Address the letter to the Consulate General of Canada ( not the
embassy - thats in Washington)

Hi,

I am in USA on a H1 visa

What should be option Immigration status of applicant(s) in country
where applying:

Permanent Resident
Temporary Resident
Worker

I am confused with the  Temporary Resident and Worker

When I am in USA work under H-1B, I put "Worker".

In my case before, when I lived in USA I never stay in extended
time outside USA (each trip outside USA at most few weeks), so I
put the date the *first* time I went to USA to move there, and
put for "To" date, the date I filled the form - when I still
live in USA, or - after move out from USA - the date I moved out
from USA. I did not put all the short trip between those dates.

You can always attach additional paper listing additional
information. Put down "see attachment" in the particular entry
in the original form.
