medical examination for all?

I have my family here on a visitor visa, to apply for an extension all them
need to pass medical examination, or just my wife? the kids are 11 and 2
years old
What is better, they came with my wife and are named on here admition pemit,
can I change them into mine? or their condition as a temporary residents are
linked to my wife's status?

They all require medicals.
