Incorrect Mailing Address on Medical Form - IMM 1017 - Experts Please Advise

Dear Experts,

The Mailing Address on my Medical Form - IMM 1017 is incorrect. The
address on the Envelope and on the main letter from the Consulate is
the new updated address which I had sent to the Consulate a couple of
months ago. However the address on the Medical Form has not been
updated. I am assuming that this could have been as a result of the
Medical Form being prepared very early in the process even before the
case is assessed.

The cover letter for the Medical Form says that "If there are any
errors, please advise us by mail or fax to 716 858 9591 and show your
file number."

Which of the following should I do :

- I follow the above instructions, update the Consulate about the
error and wait for the new forms and then take the Medical Test after
receiving the new forms.

- Or should I update the Consulate about the mistake and make the
change myself on the IMM 1017 form and take the medical test right
now.

- Or should I update the Consulate about the mistake and ask the DMP
to change the address error on the IMM 1017 and go ahead and take the
medical test right now.

Experts please adivse.

The medical from was probably printed a long time ago before your change of
address.  Just ask the DMP to make note of the new address on the form.

Medical forms are printed in early stage of the process (well before
assessment), so it is quite normal that they show your old address as you
changed it quite recently. Ask DMP to correct the address.
