The US Embassy in Colombia is requiring an I-134 Affidavit for Support for K1
visas. I am going to need a co-sponsor, which I already have. Can anyone tell
me what form the co-sponsor will need to fill out? Do they need to fill out
another I-134 or is it a different form?

You're co-sponser will need to make income to cover the sponsers household
AND your household... Example
Co-sponser has three in family and you have two
will need income for Five...............
I-134 is not a legally binding document and the I-864 is.
the consulates know this and the co-sponser may not carry as much weight as
you might think at this stage.
