Hi, my hubby is refiling his AOS. We went to the Dr. today and he
filled out the I-693 (we had filled out and brought copies with us to
his office but he had some pre-printed forms with Dr's info on it so we
used his).
Someone mentioned the vaccination supplement - is this a different form?
How do I know if we need it?
Hubby came over on a K-1 almost 3 years ago...

I'm thinking that it is in the sealed envelope that the doctor will give
back to you. S/he may also give you a copy for your records. When we
went to the doctor, we did the same as you...filled out the I-693 that
the office already had. When we went back to pick up, the nurse handed
us back the sealed envelope and a copy of the supplement. If you did
not take it to the medical already, take your vaccination history with
you when you go to pick up your envelope and ask them to fill out the
supplement if they haven't done so already, they should have the form.
If that's the case, they may need to reseal the envelope contents with
the supplement in it and also give a copy of it to you.

I went ahead and sent the vaccination "sheet" I was given at the medical
exam in Madrid instead of the I-693. It has been a year and 7 months..
way pass the date. I though I'd risk getting a RFE, what can I lose?, I
am here with hubby.:) Good Luck!

The vacination supplement is usually used by K1ers to prove (when they
file for AOS) that they have had all the required vaccines. Usually
they will only just have had a recent official medical (at the US
Embassy in their home country) and this supplement suffices when they
come to file for AOS.
I had my medical in London although didn't get all the required
vaccinations (this isn't a requirement at interview stage). I had them
done over here in the US and when I filed for AOS I submitted just the
supplement. You'd have to submit this as well - although as it's been
so long since your hubby entered on a K1 you'd need another full
medical anyway.
