I need to make a stat dec for my mother. She has applied for a
contributory parent visa. The stat dec needs to say her name, address,
DOB, how many children she has, marital status etc. I have tried to put
something together but i can not come up with anything good.

hi, whats the go with the medicals, i was under the impression that the
doc sent them direct to australia. on form 26, part A 12 it askes which
office i intend to lodge my application, for pommies am i right in
saying this would be the AHC in London?
